SENIOR HOUSE OFFICER IN PAEDIATRICSJOB DESCRIPTIONThe CHILDREN'S SERVICE integrates hospital and community paediatrics, including medical and nursing staff as well assupport and therapy services. It has been awarded the Charter Mark on three consecutive applications. The Service ismanaged within the local Primary Care Trust.The Paediatric Department at Hinchingbrooke Hospital consists of a 27-bedded ward (shared with surgical specialists), anadjoining Out-Patient Suite and a 12 cot Special Care Baby Unit including 2 Intensive Care cots and 2 High Dependencycots.The Children's Services Unit is housed in separate accommodation on the outskirts of Huntingdon and comprises: Child andSchool Health, Environmental Health, Health Visiting and School Nursing, District Nursing, Chiropody, Physiotherapy,Speech Therapy, Dietetics, Audiology, Orthoptics, Dental Services and Health Promotion, all of which are based on thatsite.The Consultant Staff within the Integrated Childrens Services are:Dr J Challener - Diabetes MellitusMedical DirectorDr M Becker - Growth DisordersRenal DiseaseClinical DirectorDr J Valente - Respiratory PaediatricsCystic fibrosisPaediatric TutorDr B Algawi - CardiologyDr H Dixon - NeonatologyOncology shared careDr F Latcham - GastroenterologyAllergyDr U Rajalingam - Neurology- EpilepsyPaediatric TutorDr C de Cates - Community and Social PaediatricsLooked after childrenFostering and adoptionDr L Cormack - Community Paediatrics and CardiologyDr N Johnson - Neonatology (due to start the end of 2005)Dr M Reithatha - CommunityNOVEMBER 2005There are 4 Specialist Registrars (5 year rotational posts), 1 Associate Specialist (community) 2 part-time Staff Grades(community), 5 SHOs, 2 SHO Trust Doctors, 1 FY1.The hospital neonatal unit presently deals with 2,300 deliveries a year, 220 admissions to SCBU and more than 400Intensive Care days. The neonatal unit is now within the 8 units managed by the Norfolk Suffolk Cambridgeshire NeonatalNetwork and is a Level 2 unit which manages most of its low birth weight babies in close collaboration with the unit atCambridge. Three of the consultants have a special interest in neonatology and take lead responsibility for neonatalcare.The childrens ward and adjoining out patient suite see 2,200 paediatric admissions and 6,000 outpatients yearly.Huntingdon has a young population and the Paediatric Department has seen rapid expansion in the workload.There are special clinics for children with epilepsy, neurological diseases, prematurity, respiratory problems,gastrointestinal problems, allergy, diabetes, growth- and endocrine problems, cardiology, renal and urinary tractproblems and bowel control problems. There are joint clinics with orthopaedic surgeons (including a botulinum clinic), apaediatric urologist, a respiratory paediatrician and cardiologist. There are Clinical Genetics and Child Psychiatryclinics held within the unit.THE POSTThis is a full-time post based at Hinchingbrooke Hospital, Huntingdon, Cambs (General Paediatrics, Neonatal IntensiveCare, Community Paediatrics) for 6 months. Previous experience in paediatrics is essential.The SHOs and Trust Doctors share the same responsibilities whether on the GPVTS or having an individual contract with theTrust.The SHOs and Trust Doctors at Hinchingbrooke Hospital rotate and hence experience is gained in Paediatric in-patient andout-patient care, neonatal intensive care, assessment of the healthy newborn as well as some aspect of child psychiatry,child health surveillance, child protection work and other aspects of Community Paediatrics.The post is compulsorily resident and is a full shift rota and complies with the working time directive on juniordoctors hours.SHOs with less than one years paediatric experience (GPVTS) will always have a registrar or Staff Grade as a secondcolleague on-call. Cover for SHOs with Paediatric experience is mostly with middle grade staff, but if not is provided bya Consultant Paediatrician, resident on site if necessary pending further middle tier appointments.The SHO is contracted for internal cover for annual leave and short periods of study leave. He/she will also be expected to be involved in teaching Medical Students allocated to the Department from theMedical School, Cambridge University.POSTGRADUATE EDUCATIONClinical GovernanceThere are quarterly Perinatal Mortality Meetings, monthly obstetric/neonatal ultrasound audit meetings, monthly ClinicalGovernance Meetings and annual Post Neonatal Death Meetings.The Department is participating in the following audit projects: To review management of low birth weight babies. This is ongoing regional study. To compare and review management of children with cystic fibrosis among all districts in East Anglia using locallyagreed indicators. Identification of children with Special Educational needs. The age of fitting hearing aids in sensori-neural deafness.AssessmentThere will be a personal assessment of your competence, skills and participation in teaching halfway through and at theend of your attachment. The assessment is confidential and only shared with you and your Educational Supervisor. You, inturn, will be asked to assess the post.Study LeaveYou are entitled to apply for Study Leave to attend certain courses and sit certain examinations essential for training.Study Leave is granted on application and on approval by your Consultant, the College Tutor and the Clinical Tutor inaccordance with East Anglia guidelines.The Half-day Release Course for GPVTS trainees is taken out of Study Leave allocation.Medical StudentsTwo or three Medical Students from the Medical School of the University of Cambridge and from abroad are usually attachedto our Department and SHOs are expected to be involved in their teaching.The Medical Education CentreThis is a well-equipped Education Centre with a Lecture Theatre and Seminar Rooms. It is situated adjacent to the JuniorDoctors' Mess.Clinical TutorThe Clinical Tutor is responsible for the Medical Education Centre throughout the hospital and is the localrepresentative of the Postgraduate Dean. Clinical Tutor: Dr S Lishman.Careers Guidance TutorThe Careers Guidance Tutor is an experienced Consultant who can be approached for advice as regards career plans, careerchanges or difficulties within the present appointment.
